Aging Services Coordinator

Job Description

The Georgia Department of Human Resources, Division of Aging Services (DAS) is seeking qualified candidates for the Health and Wellness Aging Services Coordinator in the Livable Communities section.

This is a full time temporary position. This position will terminate with the grant (3-30-2012) unless other/continuation funding is obtained.

Under limited supervision, serves as the Project lead at the state level for the Chronic Disease and Self Management Program (CDSMP). Working in conjunction with five Area Agencies on Aging, the Health and Wellness Aging Services Coordinator will be responsible for planning and implementation of the CDSMP, coordinate efforts with DCH, AAAs, state and local health departments and other agencies to ensure success of the program; develop a business plan for the deployment of the CDSMP program in selected regions of the state; provide technical assistance and training; attend grantee meeting; host quarterly network meetings; monitor project activities and budgets; and collect and compile data for AoA and other Statewide reports.

Responsibilities require emphasis on quality management, including analysis of programmatic, fiscal and outcome data. Oversight responsibilities for five Planning and Service Areas, including monitoring and monthly ARRA data validation and ARRA Reporting. Team leadership expectation for DAS, inter-agency steering committee to include state level public health and Medicaid.


Note: Travel is required.

Qualifications

Bachelor's degree in a related field and two years of related experience Or Master's degree in a related field

Preferred Qualifications
Preference will be given to candidates, who in addition to meeting the qualifications listed above, possess the following:
  • Registered dietician
  • Experience in GA Aging Network
  • Excellent written and oral communication skills
  • Knowledge and or experience with the Stanford model for CDSMP
  • Strong proficiency in Microsoft computer applications: Word, Excel, PowerPoint, Visio, GroupWise e-mail, etc.
  • Experience in providing consultation and technical assistance on policy interpretation and clarification
  • Certification in a field related to the area of assignment, e.g. certification in Gerontology
  • Demonstrated leadership in current work duties/ performance and skills
  • Experience in developing and conducting training
